severity and manageability scales

Just looking for any information on severity and manageability scales, in particular the descriptions or guides given to staff when reporting incidents. Any help welcome, thanks

The Checklist of Challenging Behaviours by Harris et al 1994 is an interview that has severity, management difficulty and frequency ratings on a 5-point scale. You could always use these same scales on any recording formats you already have.
